Hi Anselm
Following on from the recent Board meeting I attach a letter from the Board to yourself concerning a
purported forgery.
I also attach the following -
- the initial email thread between myself and [ s 9(2)(a) ]
when he first raised the issue with me as
Secretary;
- the form 5 containing the alleged forged signatures received from S+S NZ;
- the email thread sent from Jo Johnston (Chair S+S NZ Examinations) to Vicki (Chair CSLB) and myself,
following her verbal discussion with Vicki - who raised the matter with her. This thread begins with my
email brief to the Board on 8th February, prior to the CSLB meeting on 12th February.
I declare a conflict of interest in my capacity as a QA reviewer of [ s 9(2)(a) ]
datasets.

I previously referred to this ’complaint issue’ in my report under Item 6(iv). This is the email thread – FYI before Vicki
updates the meeting about the S+S agreement.

Hi Vicki
Here is the email thread.
My online spreadsheet shows that his form was sighted by the Examiners and therefore he was accepted
for an interview. If there was no form from him, he would have been asked for one. So, I don't think S+SNZ
under the Official Information Act 1982
correctly removed the form as stated. It is possible he submitted one form with each project and S+SNZ
only removed one form from one of the projects.
I can see I was first copied into the emails on 19 August 2020 (pre-exams) but that was an administrative
issue for S+SNZ to request a new form from the candidate (which they didn't). So, by the time it appeared
in front of the examiners it was taken at face value.

I was his supervising LCS, however I did not read his projects nor endorse him as required to pass the professional
interviews.

I would recommend an audit on his first few plans.
