Request for information from the Coroners Court file regarding Isabella Rangimohia
ALEXANDER
Thank you for your email of 5 April 2024 to the Ministry of Justice (the Ministry) requesting
information off the coronial file regarding the late Isabella Alexander. I am responding to your
request under the Coroners Act 2006 (the Act).
Your request referred to a 15 September 2021 New Zealand Herald Media article which
referenced a Coroners Court Case into the death of a teenager. Specifically, you requested:
“Kindly share for this death on 10 Sep 2021 for this teenager:
1. the date that the Coroner's investigation began;
2. the date that the Coroner's report was concluded.
3. Please include the dates for Coronial Autopsy Report and
4. the date of concluding Principal Pathological Findings. “
Based on the information provided to us in your request of 5 April, the Ministry identified the
Coroner Court file that we believe you are requesting information to be that of the late
Isabella Alexander. I have therefore outlined below my response to your request from the
information contained on the Coroners Court file for this identified case.
In response to part 1 of your request, the Secretary for Justice was notified by the coroner
that an inquiry was opened on 14 September 2021.
In response to part 2 of your request, the Ministry can advise that the coroner’s findings are
dated 7 December 2022.
In response to part 3 and 4 of your request, the Ministry can advise that the date of post-
mortem is 11 September 2021. The coronial post-mortem report, which includes the principal
pathological findings, is dated 24 September 2021.
