What health companies / health providers were used to carry out vaccinations in each
workplace of businesses involved in the Pilot program?
What locations throughout New Zealand were these vaccines given to staff / employees or
any other person associated with the companies involved in the Pilot program? and what
dates did these vaccinations occur?
What were the number of doses administered in the workplaces in New Zealand for each
batch (i.e. number of doses administered per manufacturer provided batch identifier).
What was the number of doses for each individual workplace site?
What financial incentives were given to the businesses where vaccinations were carried
out in those businesses workplace sites?”
As part of the health and disability system reforms, the functions previously held under the
Ministry have now transferred to Health New Zealand – Te Whatu Ora. For this reason, I have
decided to transfer your request in full to Health New Zealand under section 14(b)(ii) of the Act.

Based on the pilot program mentioned in the 10 Aug 2021 minutes (H202116597 Document 2
CVIP Steering Group Minutes.pdf ) , Fonterra, Mainfreight and large supermarkets were
participating in a Pilot program for covid-19 vaccination in the workplace.
Would you kindly share the information on exactly which batches and the total number of
batches dispatched to each of these workplaces on this workplace pilot program? Please
include;
1. Which companies received batches of vaccines? and what dates they were received?
2. What addresses and to whom were these batches sent to?
3. What was the expiry dates of these batches and the batch numbers?
