Dear Fire and Emergency New Zealand,

Under the provisions of the Official Information Act I request a copy of the report relating to the FENZ National Safety Notice 013/2024 where a crew became entrapped by an advancing flame front and required rescuing. This report would have resulted from an internal review of the incident with analysis of why/how it occurred, what action resulted and the recommendations to prevent similar incidents.

Thank you for your request of ​2 May 2024​, to Fire and Emergency New
Zealand requesting the following information: 

o Under the provisions of the Official Information Act I request a copy
of the report relating to the FENZ National Safety Notice 013/2024
where a crew became entrapped by an advancing flame front and required
rescuing.  This report would have resulted from an internal review of
the incident with analysis of why/how it occurred, what action
resulted and the recommendations to prevent similar incidents.

 

We have identified two documents that may be in scope of your request. We
are seeking confirmation of which document you would like to receive. The
documents in scope of your request include an operational review, and a
safety, health and wellbeing investigation. Once you have considered the
above, we would appreciate your reply as soon as possible. If no reply is
received, Fire and Emergency will consider your request on its original
merit.

Thank you for your advice. It is the Operational Report that is of particular interest relating to how/where the entrapment occurred and any analysis and recommendations to avoid this re-occurring.

I request then the Operational Report relating to the entrapment.
