Dear Accident Compensation Corporation,

It would be appreciated if you could please provide to me a complete copy of the Guide to completing file summary (PDF 59K), which was in effect as at September 2014, which was required to be referred to when ACC employees completed the ACC 012 File Summary as part of the Recover Independence Service process.

I respectfully request that since the information that I am seeking is already in PDF format and only 59K, that this Official Information is provided under urgency within the next 5 working days, being by the end of business close on 20 March 2017, for this information is required for review and appeal purposes.

Under s16(2) of the Official Information Act, my preferred way of receiving a response is by email to the address from which ACC received the original request, not by post.
